Nurse Recognized for Excellent Patient Care

Lynn Nelson has helped Knox Community Hospital deliver high-quality value-based care through unprecedented pandemic conditions.

Lynn Nelson has been recognized as a national winner of the 2022 Population Health Nurse of the Year Pioneer Award, presented by Caravan Health, part of Signify Health. The award is given to individuals each year who have demonstrated exemplary work in integrating value-based care while serving their patients, community, and the healthcare industry.

Caravan Health selected two winners, including Lynn Nelson, who has incorporated innovative approaches through their leadership, dedication, and innovation while mitigating challenges during the COVID-19 pandemic. Lynn Nelson has played an integral role in helping Knox Community Hospital demonstrate a commitment to diversity, equity, and inclusion while working to improve patient care outcomes using patient-centered, value-based care approaches.

The criterion for this honor includes excellence in the ability to address the needs of the patient while improving the healthcare team’s ability to contribute to accountable care (ACO) success. Nelson exceeded these standards.

“I am honored to receive the Population Health of the Year award and to be recognized for my efforts as our collective teams have implemented best-practice methods that have helped to engage our patients which has led to better care and improved care outcomes. On behalf of KCH and my colleagues, I am excited to accept this award and to be included among other leaders who are motivated to make a difference one patient at a time,” said Nelson.
