This is a full-time grant-funded position. This position will provide a variety of services including, but not limited to coordinating interagency and community referrals for services to assist clients with successful completion of treatment plan, working with community organizations to provide continuity of care through collaborative services and information sharing, assess qualification of benefits for health coverage, public assistance and other identified needs, and working collaboratively with a clinical team to provide comprehensive and integrated care.
Requires good communication skills, excellent record keeping and knowledge of community resources. Benefits package offered.
Associate’s Degree or higher or demonstrate a willingness to work towards this degree, preferably in the field of human services or social work.
